I am agreeing to the following terms and conditions of the Toxin Membership with Venus and Venom Medispa. 

• $199 Set up Fee due at time of registration.
• Tox Membership is $149.99 per month with a minimum commitment of 1 year required.
• Memberships will be on autopay with a valid credit card. Gift Cards are not valid towards monthly dues
• Auto bill date will be within 1-2 days of the day of the month you joined, depending on weekends and Holidays.
• After the 1 year, Membership will begin again automatically unless written notice of cancelation is given 30 days prior.
• Maximum of 160 units of Botox or 320 units Dysport (equivalent  of 160 BOTOX) for the 12 months. Once you reach 161 units, you will be charge at a rate of $12 per unit. 
• Choice of Toxin used (Botox or Dysport) is at the injectors discretion.
• No refunds on Membership dues for any reason.
• If you choose to cancel before the year is up, all used units will revert to full price, you will be responsible for the balance. The balance due will be charged to card on file.
• You can NOT gift any units to another. 
• 160 units (or Dysport equivalent) is a maximum, not the standard. You will not be issued any credit for difference if used less than 160.
